Trips of less than 1/4 mile will
not be reimbursed/pre-arranged unless the individual is unable to walk the
distance
Ambulance transportation
and transportation to sheltered workshops, day care programs, mental health
social rehabilitation programs, WIC programs, and VA medical centers are
NOT reimbursed/pre-arranged through the Medical Assistance Transportation
Program
Exceptional transportation requests
or transportation outside the established service area will not be reimbursed/pre-arranged
through the Medical Assistance Transportation Program
Trips for any NON-Medical Assistance
compensable service will not be reimbursed/pre-arranged through the Medical Assistance Transportation
Program
Any individual whose request for services has been denied,
reduced, or terminated will receive written notification and information on
their right to request a Fair Hearing from the Department of Public Welfare.
