To be eligible an individual must:
Be a permanent or temporary Jefferson County resident
Have a valid PA Access card
Apply for services and be determined eligible every month
Need non-emergency transportation to get to and from a medical provider for a service that is covered by the Access Card
 
 Submit specific documentation for each reimbursable trip by the fifth day of the month following the date services were received
